During a routine inspection

We used a number of different methods to help us understand the experiences of people using the service. This was because most of the people using the service had complex needs which meant they were not able to tell us their experiences in a meaningful way. We observed staff interaction with the people who used the service and we spoke with three relatives of people who used the service.

People's needs were assessed and care and treatment was planned and delivered in line with their individual care plan. We observed staff carrying out care tasks and we found they were doing so in accordance with each person's care plan.

Throughout our visit, we saw that staff treated people with dignity and kindness. We observed that a friendly and positive approach was used, to ensure that the wishes and needs of people using the service were respected.

People who used the service were protected from the risk of abuse, because the provider had taken reasonable steps to identify the possibility of abuse and prevent abuse from happening.

There were enough qualified, skilled and experienced staff to meet people's needs. However, some of the relatives we spoke with raised concerns about the use of agency staff, as they thought they did not know the needs of their relatives.

The provider had an effective system to regularly assess and monitor the quality of service that people received.

During an inspection looking at part of the service

We looked at two outcomes to check whether the provider had complied with compliance actions that we issued in June 2012.

We saw that people's records were detailed, accurate and fit for purpose. They were regularly reviewed to ensure that they remained relevant and met people's needs.

There were appropriate arrangements in place for managing medicines. Staff understood their responsibilities and could describe their role. Storage and stock control arrangements had been much improved, although we identified a small number of errors which we discussed with the registered manager on the day of the inspection.

During a routine inspection

The people who were present on the day of the inspection were not able to communicate verbally with us. We used the Short Observational Framework for Inspection (SOFI). SOFI is a specific way of observing care to help us understand the experience of people who could not talk with us. Using this we saw that people experienced care from staff who took time to understand their needs and preferences. We saw staff delivering care at a pace which people appeared to be comfortable with. From our observations we saw that staff treated people with respect and acted in a way which upheld people's dignity and privacy.
