Monitoring and improving outcomes

Score: 3

The provider routinely monitored people’s care to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

As part of the assessment process staff asked people what they wanted to achieve and experience. Staff supported people to meet these goals. This included supporting people to maintain their independence as much as possible. Staff celebrated people’s talents and achievements. One person was a musician, and the provider purchased a keyboard so the person could continue to perform for people. Another person was a bird watcher. The service is set in large grounds, and the staff bought some bird books so the person could mark down what they saw and they were informing other people and staff about the nature around them.

Staff were aware of people’s needs and gave physical and emotional support to ensure positive outcomes and good experiences for people. For example, one person was often distressed but staff found that by spending time with them and holding their hand, this helped to comfort and reassure them.
