Monitoring and improving outcomes

Score: 2

The provider did not always routinely monitor people’s care and treatment to drive continuous improvement. They did not consistently ensure that outcomes were positive and consistent, or that they met both clinical expectations and the expectations of the people receiving care.

Staff knew people well and were able to readily identify changes in their needs. However, the records in place were not always sufficiently robust to evidence the actions taken in response to those changes, or the measures staff had implemented to ensure people remained safe. This is discussed further in the Well-led section of this report.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.
