Date of Assessment: 30 June – 18 July 2025. Broughton House Residential Home is a care home which provides accommodation and personal care for up to 30 people. At the time of our assessment site visit 30 people were living at the home.

We carried out a site visit to the home on 30 June 2025, the remainder of the assessment was conducted offsite. The assessment was prompted due to an aged rating. A decision was made for us to inspect all 34 quality statements across the 5 key questions we asked was the service safe, effective, caring, responsive and well-led.

The provider was in breach of regulations in relation to safe care and treatment and good governance. The overall rating is requires improvement.

Infection prevention and control practices were not always safe increasing the risk of the spread of infection. Areas of the home and items of equipment were unclean. Wheelchairs and commodes were stained with spillages and food debris. Opened and pre prepared food items held in the fridge in the kitchen were not labelled identifying the prepared date and used by dates. Containers storing food items including cereals and biscuits were visibly unclean and did not display a use by date.

Parts of the environment were in disrepair posing a risk to people’s safety. This included damaged flooring on corridors and in bedrooms across all 3 floors and damaged paving and fences in the garden area. The provider lacked oversight of the safety of the environment and failed to assure us of the action they proposed to take to make the required improvements.

Events such as accidents and incidents were listened to and investigated in a timely way and lessons were learnt to prevent future occurrences. The registered manager and staff knew the different types of abuse and how to report any safeguarding concerns. Allegations of abuse were reported promptly to the relevant agency. Medicines were safely managed by suitably trained staff. There were sufficient numbers of staff suitably skilled and experienced, and they were recruited safely.

People’s needs were assessed, using recognised assessment tools and a care plan was developed detailing people’s needs and how they were to be met. Care plans were routinely reviewed and updated each month or sooner if needed. People’s nutritional and hydration needs were understood and met, and they were given a choice of healthy food and drinks. People received the support they needed to maintain good health and well-being. Decisions made on behalf of people who were assessed as lacking the capacity to consent to care and treatment were made in line with the Mental Capacity Act 2005.

People were treated with kindness and compassion and their privacy and independence was respected and promoted.

Staff recognised and responded to changes in people’s needs and made the required referrals to other health and social care professionals. Information about the service and how to complain was made available to people and others.

There was an experienced registered manager in post, and they promoted a positive culture based on listening, learning and trust. The registered manager carried out audits and checks and maintained detailed records evidencing them. Staff felt supported, valued and able to speak up with confidence. There was good partnership working with others in ensuring people’s needs were met.

People's experience of the service

People felt safe, happy and well looked after and were treated with kindness and respect. People’s comments included, “Oh yes I do feel safe here, if I didn’t I would get help from the staff” and “Oh they’re all very nice and caring and I feel respected.” Family members told us, “The staff seem to be very kind and caring to [relative] and I can’t fault the place” and “Yes I feel [relative] is safe here.”

People were given choice about how their care and support was provided and they felt staff knew them well. One person told us, “I can get up when I want to and they help me to get ready and sit in my chair.” People told us staff were good at making sure they received the right support with their health and well-being. Their comments included, “They [staff] are kind if I’m unwell” and “I see the doctor if I don’t feel well, they ask me what I want to do but I like staying here in my room.”

People were given a choice of food and drink which they enjoyed. One person told us, “The food is very nice” and a family member told us, “The food from what I see is okay and my [relative] always eats it.”

Family members felt the home was well managed well. One family member said, “The manager is approachable, and I would be able to say if things weren’t right, I’m happy with the way the place is run.” Another family member said, “I do think the Home is well led.”
