We took enforcement action and put conditions on the registration for Whitelodge Alveley Limited on 02 March 2026 to ensure improvements are made with the services governance and management oversight at Appletrees Care Home.

Well-led
Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to inadequate. This meant there were widespread and significant shortfalls in service leadership. Leaders and the culture they created did not assure the delivery of high-quality care.
Not enough improvement was made during this assessment and the provider remained in breach of legal regulation in relation to good governance.

Capable, compassionate and inclusive leaders
The provider did not have inclusive leaders at all levels who understood the context in which they delivered care, treatment and support, or who embodied the culture and values of their workforce and organisation. Leaders did not have the skills, knowledge, experience and credibility to lead effectively, and they did not do so with integrity, openness and honesty.
The provider and leaders had continued to fail to make sufficient improvements following concerns identified at our previous 2 assessments. Quality assurance and monitoring systems were not effective in identifying and addressing issues relating to the safety and quality of the service. As a result, concerns identified at previous assessments had not been fully addressed, and management oversight had not been robust enough to drive and sustain the improvements required. This meant leadership arrangements had not consistently ensured people received safe, high-quality care.
More recently, the provider had taken positive steps to strengthen leadership within the service through the appointment of management consultants and a manager to oversee improvements in the absence of the registered manager. At the time of our assessment, the manager had only been in post for 3 weeks. However, they demonstrated the skills, knowledge and experience needed to drive the improvements required at the service.

Staff spoke positively about the new manager and described feeling supported and listened to. They told us there was a greater management presence within the home and that changes were beginning to be introduced.
