Our view of the service

Date of Assessment: 10 April to 24 April 2025. The service is a nursing home providing support to older people, including people living with dementia. We carried out an inspection looking at all the quality statements for safe and well-led and one quality statement in caring and two in responsive. Our decision to inspect the service was due to concerns received from the local authority contracts, commissioning and safeguarding teams. At our last inspection the service was rated requires improvement and was in breach of regulations. At this inspection we found serious concerns relating to management of risk, management of medicines and governance. The provider continued to be in breach of regulations 12 and 17. Additionally, we found some areas of the service were not well maintained and unable to be effectively cleaned, lack of Personal Protective Equipment (PPE), and staff training and supervision which were not up to date.

People were not protected or kept safe. There was a lack of information, and conflicting information in people’s plans of care, putting people at risk of harm. The service did not make sure medicines and treatments were safe and met people’s needs, capacities and preferences. There was lack of effective governance and oversight. There was a new management team, who understood their duty of candour, to be open and honest when things went wrong. They were committed to working with health care professionals to try to improve the service. However, the provider did not learn lessons or maintain improvements to ensure the service improved to good.

We have rated this service inadequate, and this service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.

People's experience of this service

People told us they were happy living at Lower Bowshaw View, we observed positive interactions between people and staff. Staff were kind and caring. However, management, leadership and deployment of staff varied depending on which staff were on duty. It was noticeable when bank or agency staff were on duty that the staff were not effectively supported, which impacted on the care people received. The care records were also not up to date, which put people at risk of harm.

We identified serious concerns, that impacted on people’s care and support they received. For example, care plans lacked detail for staff to be able to deliver safe care and treatment that met people’s needs and we found medicines were not safely managed, people did not receive their medicines as prescribed.
