During an inspection looking at part of the service

The local authority and the local primary care trust have been monitoring this home since December 2010 because they are concerned about the welfare and care of the people who live there. Working together with other commissioners they told us that they have found that there had been positive developments in the general management of the home of the home with significant improvements in the way people are cared for.

We spoke with eight of the people who lived at the home. Three of the people we spoke with were able to discuss the way their care was provided but could not say or recall giving consent about the care they received. They all said that they had never seen a care plan. One person said that they 'did not know how decisions about their care were made'. They said 'life was just about getting up watching TV and going to bed'. They said that they 'did not feel well cared for and were left to their own devices'. Another person said that they 'had never seen a care plan and had never been asked about the quality of care provided'. They said that they 'did not feel involved in decision making about their care' and that they 'never get any information and are in the dark'.

During an inspection in response to concerns

We spoke with seven people who lived at the home. Some of the people we spoke with were able to discuss the way their care was provided but could not say or recall giving consent about the care they received. They told us that they did not know about care plans and had never been asked if they wanted to see their care plan.

None of the people we spoke with raised concerns about the standard of care or facilities and services provided.

During an inspection in response to concerns

We spoke with six people who lived at the home and two visiting relatives. Some people who lived at the home told us that they had little do and were bored but all said they were generally well cared.

Some of the people we spoke with were able to discuss the way their care was provided but could not say or recall giving consent about the care they received. They told us that they did not know about care plans and had never been asked if they wanted to see theirs.

The relatives we spoke with told us that they were aware that staff acted in the best interests of the person but they were not always happy with they way decisions were made. They told us decisions made did not always balance the degree risk against the impact the decision to use safeguards had on the person's quality of life. They were concerned because their relative was unable to speak for themselves and although they had been informed they had not been consulted when these decisions were made.
