Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

Everyone had a care plan, appropriate assessment and risk assessments in place. There was a mixed level of detail in care plans. Some care plans included relevant detail to ensure people were supported safely and effectively, however some care plans missed person-centred detail. For example, what people were able to do for themselves and what they needed support with or clarification on how often people needed repositioning. We raised this with the provider during the first day of our assessment, the provider advised us they would arrange further training for staff.

Delivering evidence-based care and treatment

Staff demonstrated they knew the people they supported well.Staff including the cook had good knowledge of people’s dietary needs and their nutrition and hydration preferences. We received positive feedback from relatives about food the home offered. One relative said, “It’s a good home, the meals are nice and it’s clean.” People were also positive about the choice of meals. One person told us, “The meals are lovely; you get a choice for example you can have a cooked breakfast or cereal etc. We have a main meal at lunchtime. I have a takeaway every Friday night with my friend.” During our site visits, we observed activities taking place with a number of people. We received some feedback from people that at times they were occasionally bored and there was not enough to do.

How staff, teams and services work together

The provider worked well across teams and services to support people. The service worked with other professionals across all aspects of people’s support. Staff made sure health professionals were kept informed of what other professionals had prescribed, or action taken.

Supporting people to live healthier lives

Score: 3

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. The staff took action where there were concerns, for example when people lost weight, the GP was informed. The GP also maintained regular contact with the home. We received feedback from relatives that their loved ones had improved in weight, mood, mobility and involvement in activities since living at the service.

Monitoring and improving outcomes

On the whole, people, their loved ones and advocates were happy with the care they received and felt they were supported by a group of staff who knew them well.Staff were aware of people’s day-to-day care and support needs, and the general support they needed to maintain good health.However, staff were not sufficiently aware of how to support people in situations which may cause them anxiety. Upon review of people’s care records, we found there was not a consistent approach to how staff were dealing with people who presented with aggression. We raised this with the provider who took action to address this with staff.

People were supported to have maximum choice and control over their lives. Staff supported them in the least restrictive way possible and in their best interests.People had appropriate Deprivation of Liberty Safeguards in place to ensure any restrictions in place were in their best interest. The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Staff sought consent when providing care and support.
