Our view of the service

Date of assessment: 24 March 2025 to 23 April 2025. Garlands care home is a residential care home providing accommodation and personal care to up to 23 people. At the time of our assessment, there were 19 people living at the home. This assessment was prompted by concerns we had received about the service. The service was previously in breach of the legal regulation relating to good governance. Not enough improvements had been found at this assessment, so the provider remains in breach of this regulation. During the assessment, we found audits completed did not identify the concerns we saw in relation to medicines, recording of incidents, staff files and care records. We also found there was no clear process in place for staff to record incidents which would allow learning from any incidents that occurred. We found that the service did not have a clear staff structure, some staff had multiple roles which made it difficult to determine what roles needed covering and some staff were repeatedly taken away from their allocated role to step into another role. Everyone had a care plan in place but there was mixed level of detail in care plans. Overall, people and relatives were positive about the care they received. Staff had good knowledge of people’s care needs, including the cook. We observed caring inactions between most staff and people. Staff protected people's privacy and dignity. We found no concerns in relation to administering medication but there were some gaps in the auditing process. Times of medication administration were not reviewed and neither was the use of thickeners. We found that the premises were clean and welcoming with no malodour.

People's experience of this service

We received positive feedback from relatives about food the home offered. We received feedback from relatives that their loved ones had improved in weight, mood, mobility and involvement in activities since living at the service. People told us they were supported in a way they wanted and staff respected their choices. Feedback was also received from relatives in relation to staffing levels which was not overall positive.
