Condition of this registration relating to carrying out this regulated activity
A. The Registered Provider must not admit any new service users to Bryony House unless a comprehensive assessment of their needs has been completed, and this assessment must directly inform the development of individualised care plans and risk assessments, all of which must be fully documented and in place prior to admission
B. The Registered Provider must immediately review and reassess any changes to service users’ needs or risks, including in response to an accident or incident.
C. The registered provider must ensure service users’ care plans and risk assessments contain complete, accurate and up to date information regarding service users’ needs and associated risks to ensure staff have guidance to follow.
D. The registered provider must ensure regular and effective audits are in place to highlight risks and enable improvements at the service.
E. The registered provider must create a monthly action plan with updates resulting from an effective auditing process. This action plan must have clear objectives which are measurable, achievable and reviewed to ensure completion.
F. The registered provider must forward a copy of the monthly action plan to the Care Quality Commission on the first Monday of every month.
Terms of this registration relating to carrying out this regulated activity
The Registered Provider must only accommodate a maximum of 35 service users at Bryony House.
The Registered Provider must not provide nursing care under Accommodation for persons who require nursing or personal care at Bryony House.
