During a routine inspection

Gordena Care Home provides accommodation, personal care and support for up to 9 people. There was one vacancy at the time of the visit. People who live at the home have a learning disability. There were seven single bedrooms and one shared bedroom. The kitchen and dining room were situated on the third floor which was accessed by stairs. There was a stair lift that could be used by people if required to reach the second floor where the lounge was situated.

There was a registered manager in post. The registered manager was also one of the owners of the business. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

This was an unannounced inspection, which meant the staff and the provider did not know we would be visiting. The inspection was carried out by one inspector on the 14 and 16 April 2015.

People were protected from the risk of abuse because there were clear procedures in place to recognise and respond to abuse and staff had been trained in how to follow the procedures. Systems were in place to ensure people were safe including risk management, checks on the environment and safe recruitment processes. People received their medicines safely.

People’s views were sought through care reviews, house meetings and surveys. People’s views were acted upon. Systems were in place to ensure that complaints were responded to with action taken to improve the service provided.

People were involved in making decisions about their care. People had a care plan that clearly described how they wanted to be supported. People had opportunities to take part in activities both in the home and the local community. People were encouraged to be independent. Some people could access the community on their own whilst others required staff support. Some people managed their own finances. Other health and social care professionals were involved in the care of the people living at Gordena Care Home.

Staff were knowledgeable about the people they were supporting and spoke about them in a caring way. Staff had received suitable training for them to deliver safe and effective care. Staff told us they were supported in their role and met with the registered manager regularly to discuss their performance and any training needs.

The service was well led. There was evidence that learning took place from incidents, accidents and complaints. The provider was a member of various organisations which ensured they were kept up to date with any changing legislation relevant to the care industry. They completed regular checks on the systems that were in operation in the home to ensure they were effective. The organisation’s values and philosophy were clearly explained to staff and there was a positive culture where people felt included, and their views were sought. There was an emphasis on Gordena Care Home being people’s own home.

During a routine inspection

At our inspection on 31 October 2013 we found that the provider had not ensured that fire training for new staff protected the safety of people living at the home. We found that some areas of the home had been poorly maintained.

When we returned on 30 January 2014 we found that improvements had been made. We spoke with two staff on duty who demonstrated a clear understanding of fire procedures and how to support people. On the day of our inspection the maintenance had not been fully carried out and the provider undertook to complete this within eight days. We have since received evidence of this from the provider and are satisfied that the work has been carried out.

During a routine inspection

People living at the home told us they were happy, felt safe, and had good relationships with the staff. We observed that staff were warm, friendly and respectful when they interacted with people.

People received individualised care tailored to their needs and were supported to access the community independently where appropriate. People had detailed care records which assessed their care needs and guided staff on how to ensure these needs were met.

People had enough to eat and drink and staff were aware of their likes, dislikes and any other needs. People were very happy with the meals at the home. Where people had specific needs a speech and language therapist had been consulted to guide staff on how to provide suitable support.

The premises were safe and suitable for the needs of people however we identified some areas where maintenance was outstanding. When one person had found the stairs difficult to climb, the provider had installed a stair lift to enable them to access the communal lounge on the first floor.

Staff received suitable training and supervision however the provider had not ensured that all staff knew how to support people safely in the event of a fire.

The provider regularly monitored the quality of the service by seeking feedback from people using the service, their families, health and social care professionals and staff. Feedback from these sources was positive about the service provided.

During a routine inspection

When we spoke to people about the support they received from staff they told us that the staff were polite, caring and friendly. One person told us; "I get on well with the staff they are all really nice", another person said: "I don't have any complaints, the staff are very kind and caring when they support me".

People were encouraged and supported to make day to day decisions about their life. For more complex decisions and where people did not have the capacity to consent, the provider had acted in accordance with legal requirements.

The provider and staff were extremely knowledgeable about the practical and emotional support needs of the people in their care. We saw that staff were friendly and professional when supporting individuals at the home.

People living at Gordena Care Home can be assured that the administration, recording and handling of medicines was done in a safe manner. People were supported to raise any concerns they may have about their care or the services being provided at Gordena Care Home.

During a routine inspection

People who spoke with us were able to discuss their life at the home and what they

enjoyed about living there.

People said that they appreciate having their own rooms, and being able to choose how

they are decorated. One person told us "I chose the things I like to have in my room

People told us that they liked living at the home. One person said "I've lived here for ten years. I really like it. It's a decent home. I love it here"."

People told us that they made decisions about what they did during the evening. One

person told us "we can choose what we do. I like to watch the soaps on the television."

One person told us how the staff has helped them to learn how to get to college or to daycentres. One person told us 'we all go together in the minibus". They also told us that they assisted the staff prepare meals in the home.

Four people who use the service told us that they felt safe at the home and that staff are 'kind and considerate".

People told us that the food was "tasty" and that there was plenty to eat at times that

suited them. One person said "we decide what we want to eat. I don't like faggots so the staff make me sausages".

People told us that there was assistance for them to maintain their personal hygiene if

required and that their privacy and dignity was respected. One person said "I can always

have a quiet chat with the staff. We can go to my room or I talk to them in the office".
