Involving people to manage risks
The provider did not always work well with people to understand and manage risks. Staff did not always provide care to meet people’s needs that was safe.
At our last inspection, we found concerns with the management of risk and told the provider to take action. At this inspection we found the necessary improvements to keep people safe had not been made.
We again found when people had been identified at risk of choking, the provider had not taken the required action to keep them safe. Records confirmed they continued to receive a diet that was not safe which placed them at risk of choking and harm. We found a third person had been identified at risk of choking by a visiting professional since our last inspection. We saw the provider had not followed the advice given by the visiting professional to keep this person safe. This placed this person at risk of harm.
Following our last inspection the provider had admitted a new person into the service. The provider had not adequately assessed this person’s needs prior to their admission, therefore they did not have the equipment they needed to keep them safe on arrival. We found the person did not have adequate pressure relief and remained in bed as equipment was not available to enable them to mobilise safely. This placed the person at risk of sore skin and harm associated with being confined to their bed.
