During an inspection in response to concerns

One person said, 'I like living here, the staff are good to me.'

Another person told us how they wanted to move into supported living accommodation but this was a big step for them. We asked this person if they had worked with the staff to put together a care plan to help them become more independent in preparation for moving on from the care home. This person was not aware of any discussions with staff about this and they did not appear to know about their plan of care.

One person said, 'its good here, I can come and go as I please. I go out very early and then come back for my meals.' We asked this person if staff ever went out with them and they told us staff never went out with them, they always went alone.

One person who had diabetes said, 'they give me the same food as everyone else, but they give me smaller portions. At first they made me have just yogurts, but they did not fill me up and I was hungry.'

We observed that people using the service were living in dirty and unhygienic conditions.

One person using the service told us, 'I can smell the bathroom (situated down the corridor) from my bedroom, it's not very nice.'

One person said, 'Its pretty filthy here and it smells.' Another person said, 'It's clean here, the staff clean.' We saw a mattress for one person that was heavily stained. This person said,' it's been like that since I moved in. I did not do it.'

One person using the service told us,' I can smell the bathroom (situated down the corridor) from my bedroom, it's not very nice.'

Another person told us, 'when I use the bathroom and go to put paper towels in the waste bin the bin is always full of incontinent pads that have poo on them, it smells and is horrible.'

We spoke with a service user who had limited mobility and lived in an upstairs bedroom in the service. We asked them about a recent fire in the home and if they had been evacuated. They told us, 'I heard the fire alarm go off and I knew this must be real as the staff would have told me if it was a test. My friend came in later and said everyone had been evacuated.'

During an inspection looking at part of the service

One person using the service told us 'can you hear them (another person using the service) shouting again? 'they are always shouting, it drives me mad.' Another person said, 'It's not pleasant here because residents are shouting all the time. I feel unsafe because of them shouting abuse. The staff are alright most of the time.' One person told us 'I feel safe here. It's the best home I've been in.'

Some people told us they enjoyed their meals and others were not interested in eating.

People told us that they knew that they could find staff in the office at certain times of the day but other people told us they had to search for staff at other times. Relatives of people who use the service told us 'the staff just seem to be floating around, we have never seen any activities being done, and people appear to be left to their own devices'.

People who use the service told us that the cleaner was always around during the day and they told us that their own bedrooms were cleaned once a week.

The relatives of two people who use the service told us 'the whole place is run down, it could all do with a good upgrade, everywhere is so shabby'.

During a routine inspection

People we spoke with knew that the staff had written plans for their care. People were offered help with personal care. Some refused to have baths or showers and some people chose to wear night wear during the day.

One person said, 'All we do here is eat and drink and it's a long time to the next meal.' Another person said they sometimes had a game of table football or bingo. There were records of activities people had previously taken part in. One person told us of daytrips some people had last year and these included a safari park, Skegness and Matlock. Five people had been to Goose Fair last year. It was not possible for all people to go into the community, as there was not always enough staff to accompany them.

Most people told us that they liked all the meals that were provided, but some people refused to eat. People told us they felt safe at the home and that they would tell staff if they had any concerns.

Some people liked the premises, but were struggling with the physical layout of the home due to ageing and disabilities.
