During an inspection looking at part of the service

We considered all the evidence we had gathered under the outcome 'records'. This helped us to answer one of the five questions we always ask:

' Is the service safe?

We found that the provider (Cotebrook Limited) had taken appropriate action to improve records. This helped to ensure people were adequately protected from the risks of unsafe or inappropriate care and treatment.

We also noted during the inspection that the service had been operating without a registered manager since May 2013. The Care Quality Commission will be taking action to address this matter.

During an inspection looking at part of the service

During our visits to the service in July and August 2013 we identified several areas of non- compliance. The purpose of this visit was to assess what actions and improvements had been made by the provider to establish compliance.

During our visit we saw that improvements had been made in relation to the management of medicines; the environment; supporting staff and assessing and monitoring the quality of the service.

We found that further improvements were needed in relation to records.

During an inspection in response to concerns

We saw that improvements were needed in relation to medicines management. At the time of our visit the manager of the service was in process of reviewing and updating the policy and procedures relating to the safe management of medicines.

We saw that recruitment procedures were in place in relation to new staff being employed at the service.

We saw that improvements needed to made in relation to what training was available for the staff team. The manager of the service was in the process of addressing these shortfalls.

During a routine inspection

People told us positive things about the service they received at Cotebrook House. Their comments included 'it's absolutely fabulous, the staff cannot do enough for you, you only have to ask, it's like staying in a hotel'; 'very nice'; 'it's all on first name terms here and I've lived her now a long time by choice' and 'we go on lots of trips including going to the theatre, I like to join in the activities.'

Since our last inspection we saw that some improvement had been made in relation to the content of daily records. However, during this visit we identified that improvements were needed in relation to the environment; assessing and monitoring the quality of the service and records.

A new manager had recently started in post at Cotebrook House. The manager told us that they were in the process of auditing systems within the service and compiling an action plan for areas in which they felt they could make improvements to the service.

In this report the name of a registered manager appears who was not in post and not managing the regulated activities at this location at the time of the inspection. Their name appears because their registration was in the process of being cancelled at the time of our visit.

During a routine inspection

We spoke to seven people who use the service, they told us that were able to change the way their care was delivered. Their comments included 'people will do things differently if you ask them to' and 'you can always ask to change things.'

People spoken with told us that they were happy with the care and support they received. Their comments included 'very happy with the care I receive and 'staff look after me well. Quite happy.'

People told us that they always had a choice of whether they attended the activities available.

People told us that they had regular access to their GP. One person told us 'they will always contact your doctor if you ask.'

People spoken with told that they enjoyed the food served and that they always had a choice of what they wished to eat. Two people told us that they chose to sit in the small dining room and they commented that 'the food is very good' and 'the 'food is always nice.'

People's personal records were not always detailed or fit for purpose. For example, we saw that improvements could be made in how information is recorded in people's care plans and staff training records.

Prior to our visit we spoke to staff from the local authority who regularly visit Cotebrook as part of their monitoring of the quality of services. They told us that they had no concerns about the service delivered at Cotebrook.

During a routine inspection

During our visit we spoke to six people living at the service. They told us that they were always given choices in their day to day life at Cotebrook. People told us that they chose what time they got up in a morning and went to bed in an evening, and had a choice of what they wanted to eat.

People told us that staff supported them in a manner that respected their privacy and dignity. They told us that the manager and the staff team respected their individuality and their chosen lifestyles.

As part of this inspection process we spoke to a healthcare professional whose team supports people using the service. They told us that they thought that people's rights and dignity were respected by the staff team.
