Assessing needs
The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.
Care plans had improved since the last assessment. People’s needs were assessed and included consideration of their physical, mental health, sensory, social and communication needs.
Care plans were up to date and regularly reviewed. People were involved as much as possible in their needs assessments and felt their needs had been captured and understood. They told us, “We do have a care plan, and they do reviews but informal, they will phone and ask if there are any problems or changes. They are very good” and “[Relative] is feeling much better now. [Relative] has been receiving care from this agency for a couple of months after somebody recommended them to us. The process wasn’t difficult at all.”
Regular telephone surveys were in place where people were asked how their care was and if anything needed to be reviewed. This helped ensure there was good communication and any issues could be resolved quickly.
