At our last assessment we rated this key question inadequate. At this assessment the rating has changed to good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs. People’s risks were assessed regularly and reviewed as their needs changed. A relative told us, “I was involved in the care plan, and it has been reviewed.”

Delivering evidence-based care and treatment

The provider had a proactive and positive culture of safety based on openness and honesty. The provider had identified the recording of accidents and incidents did not give them the detail they needed to make improvements and learn from incidents. They worked with staff to improve their competence in recording which had enabled managers to accurately review incidents for patterns and themes to help prevent future occurrences.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. People’s care records held all the important information needed when moving between services such as hospital. A visiting health professional told us that staff were very good at sending all the information they needed to make their assessments.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. The nursing staff monitored people’s health and well-being through clinical observations, monitoring people’s weight, skin integrity and behaviours; they made prompt referrals to health professionals. They liaised with the GP who told us, “Together we are trying to reduce hospital admissions.”
