There were some shortfalls in how the provider understood the diverse health and care needs of people and their local communities, so care was not always joined-up, flexible or supportive of choice and continuity.
Since the last assessment the provider had reviewed some care documentation to include details of healthcare professionals involved in people’s care. This meant staff would know who to contact if the need arose.

Listening to and involving people
The provider did not make it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff did not involve people in decisions about their care or tell them what had changed as a result.
At our last assessment we found systems were not in place for people to share feedback, or raise complaints about their care, treatment and support.
The provider said they intended to develop documents and systems for people to provide feedback about their care but was not sure how they would manage this. They had not made any progress with this at the time of our assessment.

The provider made sure that contact details were in place for people to access the care, support and treatment they needed when they needed it.
Information about how to access health and social care professionals had been added to people’s care documentation. This meant staff would know who to contact if the need arose. We were not able to assess if this had been effective as the service had not been providing care and support to people.
