At our last assessment, we rated this key question requires improvement. At this assessment, the rating has changed to good. This meant people’s needs were met through good organisation and delivery.

Person-centred Care

Score: 3

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.

Peoples’ individual living circumstances and needs were now fully respected by the service. Some people had active lives outside of their homes and the service designed care around their individual needs. For example, one person required staff to be able to drive them to work and this was always provided by the service.

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity.

Staff understood people’s cultural needs which they served. Where people wished, they were supported to access their faith. Such as attending local churches.

Providing Information

Score: 2

The provider supplied appropriate, accurate and up to date information. However, they had not previously considered the value of delivering information in formats tailored to the individual’s needs. The service leaders told us they plan to provide information more tailored to individuals needs in the future. The service’s computer system had the ability to translate information as necessary into other languages.

Listening to and involving people

Score: 3

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result.

The service has a complaints investigation procedure and this had been used effectively when necessary.

People who used the service were surveyed each year to see what they thought of their service. The results from 2024 showed 95% of those who responded were happy with their service and thought it was either good or very good.

Equity in access

The provider made sure people could access the care, support and treatment they needed when they needed it.

People’s planned needs to be driven to activities and people’s immediate wishes to travel by train as a wheelchair user were supported and organised by staff ensuring people had equal access to community facilities.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.

Staff received training in equity, diversity and human rights. Staff told us about how they took people out to support their equity of experience in the community.

Planning for the future

Score: 3

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life.

The service supported people to manage (Do Not Resuscitate) DNR and Treatment Escalation Plans (TEP) so that people’s advance end of life plans were known and respected. Staff were aware of these plans and were instructed to advocate for the person’s right to have these respected in urgent situations.
