Date of assessment: 18th September 2025 to 19th September 2025. Schoen Clinic is a care home providing personal care for a maximum of 30 younger and older adults, who may have physical disabilities, mental health support needs and/or live with dementia. At the time of this assessment there were 6 people receiving care and support. This assessment was carried out in response to information of risk we had received about the service.
During the assessment we found risks associated with people’s health needs had not always been considered, identified or mitigated to reduce potential risks.
Records relating to people’s care were not always consistently completed. This included records in place to monitor people’s overall wellbeing and medicines records. Staff did not always receive the required training to support people’s diverse needs. Furthermore, staff felt there had been a lack of support to enable them to provide effective care.
There was a lack of effective oversight and governance systems. Because of this, concerns had not been identified by the service, which we found at this assessment. This resulted in breaches of regulations in risk management, management of medications, staffing and governance. The nominated individual told us they planned to implement a wide range of new audits and governance systems to monitor care quality and improve the service. This had been their intention prior to our visit.
We observed staff had positive relationships with people and supported them in a kind and caring manner. There was a high level of staffing, and this positively impacted people’s everyday support.
The environment was generally clean and welcoming. People appeared comfortable in communal spaces.
