Person-centred Care

Score: 3

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.

Staff were caring and interacted with people well. We heard staff checking if people were feeling okay and if they had everything they needed to hand. People received person-centred care which met their needs and preferences. Care plans instructed staff how to support people in line with their needs and wishes. Senior staff regularly reviewed care plans to ensure they were up to date and reflected people’s needs.

We saw evidence in people’s care plans regarding their life history, and staff took this into consideration when planning care and support. A staff member we spoke with demonstrated how meaningful activity plans were developed for people, including pictures if these better suited people’s communication needs.

Equity in experiences and outcomes

Score: 3

Staff and leaders actively listened to information about people who were most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. Staff completed regular training in equality and diversity to understand and reduce inequalities or prejudices that affected outcomes for people. The manager ensured people’s health and social care needs were fully considered and met.

Staff worked hard to provide the best outcomes for people. One person was due to leave the home and move to a more suitable environment. This would mean easier access to the community and increased opportunities to socialise; both aspects that would benefit the person.

Planning for the future

Score: 3
