At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good.

This meant people were safe and protected from avoidable harm.

The provider was previously in breach of legal regulation in relation to safe care and treatment and fit and proper person employed. The breaches had now been met, and the provider was no longer in breach of legal regulations.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

People knew how to raise concerns with the service. People told us they had not raised any concerns and had not felt the need to.

Incident reports were now completed comprehensively, detailing what happened, when it occurred, actions taken and outcomes. Lessons learned were shared with staff and discussed during regular team meetings. One staff member told us, “They tell us about what happened and if we need to learn anything. This is done with the whole team.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

People provided positive feedback about how staff supported them to manage risks. Staff told us care plans and risk assessments now contained enough information to help them understand people’s needs. Staff also demonstrated a clear understanding of their responsibilities in managing people’s risks.

We saw appropriate information was now included in people’s care plans and risk assessments to manage risks for people. For example, risks of malnutrition were identified and documented in people’s care plans.

The provider had now introduced a repositioning chart for people who needed regular repositioning. This enabled staff to monitor how and when people were repositioned, to manage any risks associated with pressure wounds.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

We did not review staff recruitment files as there had been no new starters since the last assessment. However, leaders responsible for recruitment had now attended training on best recruitment practices and shared what they had learned. The provider had clear recruitment procedures to be followed when recruiting new staff.

The provider had now updated the application form to include start and finish dates for previous employment. This meant any gaps in employment could be verified and explored.

Staff had received training relevant to their roles, which was regularly updated. Staff confirmed they had access to ongoing training opportunities. One staff member told us, “I have all the training I need; I don’t need anything else. Sometimes they give training, and you don’t use it, but they would rather have too much, just in case.”

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

People and their families told us staff supported them well with their medicines and reported no missed doses. Staff were able to clearly describe how to support people with their medicines safely. We saw evidence of staff receiving regular medicine competency assessments and spot checks. This meant leaders could be assured that staff remained competent in administering medicines.

The provider had now implemented risk assessments for paraffin-based emollient creams, highlighting the associated fire risks. Information for variable dose medicines was now included to support staff in administering PRN “when required” doses effectively.

Audits now identified where changes were needed within medicine care plans and risk assessments, to ensure continuous improvement.
