These are the forms you need to apply to register as a new provider with CQC.
Everyone needs to complete the main form. You will need to complete additional forms if you:
- have more than one location
- have more than one nominated individual
- have more than two partners
- want to provide certain types of services.

- 1 nominated individual
- 2 partners
If you need to give details for more than this, you must complete extra forms for each one.

If you have more than one location, you can complete this form for each extra location.
Extra partners
If you have more than two partners, you can complete this form for each extra partner.
Extra nominated individuals
If you have more than one nominated individual, you can complete this form for each extra person.
Extra forms based on your service
Some applicants must provide additional information. If your service type is listed below, download and submit the correct form with your application.
Home care agencies and specialist living services (SLS)
If you want to provide the regulated activity of personal care and you are one of the following:
- a home care agency (also called domiciliary care)
- supported living service (SLS)
You need to complete our additional information for providers of personal care form.
Online primary care services
You also need to complete our additional information for providers of online primary care form.
Oral health services
You must complete this additional form if you are an oral health service.
Services for autistic people and people with a learning disability
If you are applying to run a residential service for autistic people or people with a learning disability, you must show that your service meets the Right support, right care, right culture standards.
You need to complete the additional form below to show how your service meets these standards.
Submit your application
Your application must include your:
- completed provider application forms
- related registered manager applications (if needed)
- supporting documents.
We must receive all your documents at the same time. If anything is missing or incorrect, we will reject your application and you'll need to resubmit. Resubmissions count as new applications and will not keep their place in the queue.

If your files are too large to email
The largest email size we can receive is 25MB. If your documents are larger than this, compress them into a ZIP file before sending. To compress your files, go to where you saved them on your device, select them all, right-click and choose the option to compress or zip them. The wording will vary depending on your device. Once compressed, attach the ZIP file to your email before sending.
If the email is still over 25MB, split your documents across 2 or more ZIP files and send each in a separate email. Include in the subject line the provider name, the type of application, and how many emails you are sending. For example:
- Oxtown Care Limited New Provider application 1/2
- Oxtown Care Limited New Provider application 2/2
