At our last inspection at Reaside we rated this key question requires improvement. At this assessment the rating has remained requires improvement. We identified a breach of the regulations in assessing needs at Reaside.

People at Reaside were not always involved in the assessment of their needs. People using FIRST were involved in the assessment of their needs and care planning.

Staff monitored people’s physical health needs.

Assessing needs

At Reaside people were not always involved in the assessment of their needs. We looked at 7 people’s records which included an assessment of their needs. 6 did not show the involvement of the person. 5 of the 7 people’s records did not fully reflect the person’s physical, mental, emotional and social needs.

Staff monitored people’s physical health needs, and a clinic was held on site at Reaside. Records showed staff referred a person to a diabetic clinic and we observed the person thanking staff for this.

At FIRST people were involved in the assessment of their needs and support was provided where needed to maximise their involvement. Records showed that people and their family, where appropriate, were involved. Staff demonstrated how the electronic patient record system now used involved the person throughout an assessment and review of their needs. Assessments considered the person’s physical and mental health, care and wellbeing needs to enable them to receive care or treatment that has the best possible outcomes. However, staff spoke about a gap in the service where they do not have access to a speech and language therapist to ensure people’s communication needs are always met. Assessments were up to date and staff understood people’s current needs.

Staff at Reaside did not always have access to the information needed to deliver people’s care, treatment and support. 5 of the 7 records we reviewed did not fully reflect the person’s physical, mental, emotional and social needs. However, staff did have information at handovers at the beginning of each shift about people’s risks.

People’s records reviewed at FIRST fully reflected their physical, emotional, social and financial needs. Staff told us they had access to the information they needed to appropriately assess, plan and deliver people’s care, treatment and support. Staff said and records showed that when people received care from other teams or services it was coordinated effectively.
