Governance, management and sustainability

We reviewed governance process at the service, and saw that senior staff were compiling reports in areas such as incidents, use of seclusion and debriefs. Staff then produced action plans that identified what staff needed to do to improve. The action plans identified who should complete the actions. Staff updated the reports monthly and recorded progress on the actions within the reports. Staff told us that roles and responsibilities were clearly defined. Staff also told us that any learning identified by them was shared during the safety huddles so they could update the risk assessment immediately. We observed a safety huddle and saw that staff reviewed and updated risk assessments in line with the policy. Staff told us that debriefs were embedded in practice and that senior staff audited the paperwork to ensure the debriefs took place following every incident. We reviewed two PBS plans and saw they had sections instructing staff how they could offer people a debrief after they had been distressed. We saw staff used augmented communication systems with people when providing debriefs to assist people to communicate their feelings.
