Governance, management and sustainability
We were not assured that comprehensive assurance systems and processes were in place, and regularly reviewed to help manage risk within the service. For example, staffing shortages, were not recorded as incidents. The provider lacked oversight of this and could therefore not demonstrate they had proactively identified and responded to all risks and assessed the impact on the service provided. The lack of incident reporting, coupled with staffing shortages, including those in senior roles, meant that the provider could not be assured that they had a full and accurate picture of the risks and the impact it was having on the service and care provided for the patients.
We found that there had been absences in some senior leadership posts for approximately 3 months and that the provider had not ensured full contingency plans for this period. Since this had been addressed, progress has been made in some areas such as patient healthcare forums and staff told us that they felt more supported by managers.
At our previous inspection we found that not all information given to the inspection team was accurate. At this inspection we were assured that data supplied about staff training in relation to level 3 safeguarding adults and hospital life support were correct.
