Learning culture

Score: 4

The provider had a strong proactive and positive culture of safety, based on openness and complete honesty. Staff actively listened to concerns about safety and thoroughly investigated and reported safety events. Lessons were always learnt to continually identify and embed good practice. The provider carried out detailed falls analyses to identify patterns and triggers. This resulted in development and implementation of personalised falls prevention plans covering environmental adjustments, strength and balance exercises incorporated into daily routines, technology-assisted monitoring to improve staff response times and referral to the local falls prevention team for specialist input. This resulted in a significant reduction in falls for 1 person which increased their confidence in walking and improved their participation in activities. This person had not had any hospital admissions for fall-related injuries since intervention.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. Hospital care packs were prepared when people visited hospital visit. This ensured the most up to date information about people’s care was available to health and care professionals so continuity of care and support was maintained for people and outcomes improved. When we spoke to relatives about safety in the care home, one family member told us, “[It is] 100% safe. It’s the way [the staff] know each individual’s needs. [The staff] are aware of people’s changing needs and they regularly review [their needs.]” Another family member told us, “Since being in Moorgate Hollow, [relative] has not been in hospital once.”

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Safeguarding incidents were analysed by the manager. Appropriate actions were put in place and outcomes reviewed. Staff knew how to report concerns about safety to keep people safe.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs which was safe, supportive and enabled people to do the things which mattered to them. Risk assessments were completed by the provider to ensure people’s care and support was delivered safely. Where risks had been identified as increasing, the provider had sought clinical advice and there was clear information about the actions staff needed to take to minimise the risk.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. The provider reviewed key information about the care environment and facilities through monthly reports which provided detailed assurance about the safety of the care home.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care which met people’s individual needs. Staff were kind, caring and supported people safely. Safe recruitment practices were followed and pre-employment checks were completed for all staff.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. The registered manager was an Infection Prevent and Control champion and attended local area meetings to learn and share good practice. The care environment was clean and hygienic and there were supplies of personal protective equipment used appropriately by members of the staff team.

Medicines optimisation

The provider made sure medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. Medicines were managed safely. Staff knew how to report medication errors and sought clinical advice and guidance to ensure people were safe.
