At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people were safe and protected from avoidable harm.

Learning culture

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. For example, medication audits identified that as required medicine protocols did not contain enough information, and this was shared with staff at a team meeting to ensure good practice moving forward. Lessons were learnt to continually to identify and embed good practice. Staff told us lessons were learnt to mitigate future incidents. One staff member said, “Yes, they tell us about lessons learnt in our supervisions or in announcements to learn from incidents.”

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Staff completed their safeguarding training and knew how to report any concerns. One staff member said, “It’s keeping people safe from all different kinds of abuse. We would always report any abuse. I would report it to the managers in the office.” People told us they felt safe with the staff that supported them. Comments included, “Yes, the staff are trained, and everyone is ok with what [Name] needs” and “They are very good. I’m very satisfied with them.”

Involving people to manage risks

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. During the assessment process people’s environment was discussed and risk assessments put in place when needed. The service only used equipment that had been provided by health professionals and was safe to use.

 

Safe and effective staffing

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. For example, one person had a comprised immune system, and staff were asked not to visit if they have a cold. Staff were provided with enough Protective Personal Equipment (PPE) for each of their visits and knew how to use this and dispose of equipment once their visit had finished.
