Effective
Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs
The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People’s needs were assessed by the provider prior to care packages commencing. People, their families and professionals were involved, which ensured they reflected the care required. Assessments were holistic and considered the person’s health, care, wellbeing and communication needs and preferences. This promoted a personalised approach and supported positive outcomes for people. Reviews were carried out regularly at agreed intervals or when people’s needs changed. Incorporated into the reviews were the consideration of potential risks, such as the risk of social isolation. This meant care plans were kept current and reflective of people’s needs. The provider kept the service’s capacity under review to ensure new packages of care did not compromise the care of people they already supported.

Supporting people to live healthier lives
The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. People’s healthcare needs were identified in their care plans. This included the level of support thy needed to maintain good health. People and their families were confident staff were responsive to changes in need and took appropriate action. Staff and leaders shared examples where they had recognised people’s needs had changed and their care needed adjusting. Changes in people’s healthcare needs were communicated effectively with staff via the electronic care planning system, which enabled updates and key changes to be shared promptly. Where people had specific health conditions, staff had training in line with these conditions and the clinical lead worked closely with people, their families and other healthcare professionals to ensure people were supported to manage their conditions safely and as they wanted to. This helped to ensure positive outcomes for people and ensured staff had a good understanding of people’s healthcare needs and how to respond appropriately when their needs changed. Healthcare professionals told us there was good communication with the provider and they were kept updated about any relevant information or changes, which enabled them to provide timely support and guidance as necessary.
