People were protected and kept safe. Staff understood and managed risks. The facilities and equipment and systems met the needs of people. Risks were identified and acted promptly to maintain patient safety.We looked for evidence that people were protected from avoidable harm. Staff understood the importance of keeping people safe, and how to escalate any safety concerns.

At the previous inspection, we found there was concerns about the safety and oversight of the new triage system. There was limited assurance that outcomes generated by the automated triage system was safely monitored. At this inspection we noted that improvements have been made

The service worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

The practice now monitored referrals and outcomes generated by the automated triage tool to ensure patient safety and appropriate signposting. The provider had also introduced a follow up process for patients signposted to emergency services. There was now a clinical lead overseeing the outcomes and ongoing performance monitoring of the triage tool.

Where there was a lack of detailed guidance for reception staff about high-risk conditions such as sepsis and stroke. A revised triage guidance was developed incorporating the key NHS clinical red flags including common symptoms, this included physical symptoms such as sepsis, stroke, chest pain, breathing difficulties and psychological symptoms such as mental health crisis. Mandatory training sessions were held for all reception and administrative staff to ensure they had familiarity with new guidance and escalation process.
