On the whole people received personalised care that met their needs. On the nursing and residential floors this was the case. However, on the floor that specialised in caring for people living with dementia, we saw areas of practice that need improvement. People living with dementia were safe and well cared for. However, some people could have received better outcomes and personalised care if further work had been undertaken to understand their needs. This would have provided a greater equity in experiences and outcomes for people.

Equity in experiences and outcomes

Care plans contained personal information, which recorded details about people and their lives. This information had been drawn together, where possible by the person, their family and staff. People’s communication needs were assessed and recorded in their care plans. Staff were aware of people’s communication needs and how to offer them support in ways they understood. There were systems and processes followed to consult with people, relatives, staff and healthcare professionals. Meetings were carried out, providing management with a mechanism for monitoring satisfaction with the service provided.

Staff told us they knew people well, they felt they engaged effectively with them and provided person centred care. One member of staff told us, "We get to know people, we try and find out about their lives before they moved here. What kind of things they like." Another member of staff said, "The care plans have a lot of information in them about people and what they need."

People living with dementia were safe and well cared for. However, we saw that some people could have received better outcomes and personalised care if further work had been undertaken to understand their needs. This would have provided a greater equity in experiences and outcomes for people. For example, one person did not speak English. Their care plan stated they could become frustrated, as they did not understand what care staff were saying. No work had been done around enabling staff to communicate more effectively with this person. Phrases, such as ‘How are you?’ ‘Are you hungry/thirsty? ‘Are you in pain?’ could have been detailed in the care plan for staff to ask, to enable the person to indicate their needs. Furthermore, their care plan stated they were very family orientated, especially about children, but their room was sparse and did not contain any photos, mementos or personal items. We spoke with two visiting professionals, who told us staff were aware that further work could be done around improving the care for people living with dementia. They told us the management of the service and care staff were engaging positively with them to drive improvement.
