Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.

The service supported people who were no longer able to live independently due to physical limitations but who in many cases still have capacity. Several people had multiple comorbidities, with the need to engage with a range of external services to maintain good health and a degree of independence.

One person with dementia liked to walk around the service. The person’s relatives spoke to us and confirmed the home had been taking steps to maintain safe care while minimising restrictions. Staff were honest about the challenges presented by changes in people’s conditions and care needs but had suitable training and development to be prepared for this.

Care planning documents provided evidence people were being supported to live well at the service, with reviews conducted to a regular schedule and daily updates completed, so staff had a current awareness of every person’s needs. Staff therefore shared information well to enable the best experience for people living at the service.

Care was largely effective and people experienced good health outcomes. Staff responded quickly to infections, arranged GP involvement promptly, and coordinated well with external professionals. Many people saw improvements in mobility, confidence and nutrition after admission. Meals were personalised, and GP access was well organised, including weekly onsite visits
