Safe
Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question good. At this assessment, the rating has remained good. This meant people were safe and protected from avoidable harm.

Learning culture
The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were usually learnt to identify and embed good practice. Managers had reviewed procedures to further embed good practice following the specific incident. Any accidents and incidents were recorded and reported as necessary. For example, where we saw a person had bruising, records were clear about the cause, external reporting had been completed, and measures were considered to improve safety in future. A staff member told us, “They told me about incidents and accidents. They told me the process to follow. I have to report for example if someone has a fall. I have to give a full report of what happened.” The provider had implemented a new electronic system which helped managers ensure individual actions were taken and also analysed any wider themes or trends following accidents. Regular staff meetings were held and had been increased with lessons learnt discussions included. Staff told us they felt able to raise any concerns with managers. Information was available about how they could speak up within the organisation should they need to.
Safe systems, pathways and transitions
The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. Following the specific incident, the registered manager had reviewed how information was shared by external agencies, such as from hospitals, including how to deal with information which may be incomplete, to ensure people’s needs were safely met. The provider’s oversight systems reviewed when people had been admitted to hospital and the reasons for this. They worked with partner agencies to ensure people’s needs were met as their needs may change.

Involving people to manage risks
The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive, and enabled people to do the things that mattered to them. Overall, staff undertook various risk assessments in relation to people’s care plans and where necessary these were linked to their care plans. For example, one person was identified at being at risk of losing weight and dehydration, their care plan contained information about how staff should provide support to reduce this risk. Staff used equipment such as fall sensor mats and door sensors to help manage other risks. However, in a couple of examples, records needed to be more robust in relation to the actions being taken to manage and to review risks. The registered manager acted to address this straight away and shared with staff where lessons could be learnt.
