Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and, where possible, reduce their future needs for care and support.

Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff also supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. For example, a health monitoring machine was available in the waiting area of the service to enable people to carry out self-screening during their visit. The service maintained information noticeboards with posters and leaflets tailored to the needs of the local population, which were regularly updated to reflect relevant national campaigns and promote local support services. We observed information displayed in the waiting area about community groups and events, including a gardening club, social prescribing services, the ‘Fareham and Gosport Young Carers Project’, a wellbeing community group, and ‘Solent Mind’.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met all national targets for the uptake of childhood immunisations. For example, data from 2025/2026 showed the service met national childhood immunisation targets for children aged 1 with a completed primary course of vaccinations, achieving 97.1% compared to the expected 90.0% World Health Organisation (WHO) minimum acceptable target. For age 2 booster immunisations the service achieved 96.95%, compared to the WHO minimum of 90%. The provider's cervical screening rate was 80.3% in ages 50-64 and 80.1% in ages 25-49, both are above the expected 80%.
