Governance, management and sustainability
A lack of oversight by the provider had led to gaps in the identification, assessment and management of risk to appear. Whilst these had been identified by the provider, this had not always been done in a timely manner. Therefore, the inspection highlighted improvements, some of which were newly implemented, others were in progress.
The practice had a governance system that included policies and procedures. It had been identified that at times accessing these was difficult for staff and they had not always been reviewed on a regular basis. We were told that work was underway to rectify these issues.
Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations.
A data protection impact assessment was not available in regard to the use of closed-circuit television (CCTV), and we were told this would be implemented following the inspection.
There were processes in place for investigating incidents and accidents, and for receiving and acting on safety alerts.
The provider had systems and processes for learning, quality assurance and continuous improvement. However, audits had not always been completed in line with guidance. We were told that clinical assurance would be strengthened; and of plans for all audits to be repeated to enable improvements in patient care to be made.
We saw a complaints policy was visible for patients and whilst there were systems in place to manage complaints, it was not always clear how they had been dealt with and whether this was in a timely manner. Improvements were underway to ensure that outcomes were discussed and learnings shared.
Staff feedback was obtained through meetings which had been recently implemented, surveys, and informal discussions.
The practice gathered feedback from patients through online reviews although we saw that the systems in place to analyse these were not always used consistently to identify themes and patterns.
The practice had taken steps to improve environmental sustainability. For example, reducing the use of traditional dental impressions and instead using digital scanning, recycling and participating with food waste schemes.
The provider had recognised the above issues and were implementing plans to ensure that their systems and processes were strengthened and operating effectively; to enable them to continually monitor and improve the quality and safety of services provided.
