We looked for evidence that staff monitored people’s care and treatment and supported them to live healthier lives. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Feedback from care homes that worked closely with the service highlighted the supportive nature and emphasised the services culture of engagement and strong partnership working. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. The service had a newsletter that shared information that may be useful to service users such as vaccination information. The service also held get together meetings for carers to access information and support each other.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. The service performed better than the England average in relation to some cancer screening programmes, including screening for breast and bowel cancer.
The service were below the national targets for the uptake of childhood immunisations and screening for cervical cancer. The service was taking action to address these through engagement. For example, the service were offering appointments to discuss concerns with a nurse for both areas.
They ensured outcomes were positive, consistent and met the expectations of people themselves. The service had completed audits for asthma including salbutamol overuse and leg ulcers demonstrating working forwards improving outcomes for service users.
