The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff supported national priorities and initiatives to improve population health. Health promotion was embedded within the service, with accessible information in reception and waiting areas enabling people to make informed and healthier lifestyle choices. Staff took a proactive approach to identifying and managing risks to people’s health. This included early identification, enabling timely support, personalised interventions and improved outcomes.

The service was a recognised leader in environmental sustainability, achieving a Gold Award through the national Green Impact for Health programme. Sustainability was embedded within the practice’s strategy, with initiatives such as transitioning to renewable energy, delivering patient-focused wellness events and strong clinical leadership driving continuous improvement. This approach not only contributed to environmental impact but also supported healthier communities and long-term population wellbeing.
 

The service monitored all people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and fully met the expectations of people themselves.
The service demonstrated strong performance in preventative healthcare. Uptake of childhood immunisations exceeded the England average by an average of 3%, while participation in national cancer screening programmes was significantly higher than national rates, including breast screening by 12% and bowel screening by 8%. The practice also exceeded targets for urgent cancer referrals. This reflected a proactive approach to early detection, prevention, and improving population health outcomes.

Staff demonstrated a clear commitment to reducing health inequalities and promoting long-term wellbeing through their participation in the Cheshire and Merseyside Prevention Pledge. By signing up to the pledge, the practice actively supported the creation of environments that encouraged healthier lifestyles, prioritised early intervention, and embedded prevention into everyday care. This approach had led to improved patient engagement and increased uptake of preventative initiatives across the local population. Staff also outlined plans to work collaboratively with practices to promote the pledge further and increase participation across the wider primary care network.
