Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. The provider supported people to live healthier lives and where possible, reduce their future needs for care and support. The practice utilised a health and wellbeing coach and mental health occupational therapist via the Primary Care Network (PCN). The patients we spoke with praised how approachable and professional these staff members were.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Patients with long term conditions, carers and people living with a learning disability were offered health checks by the practice. The practice made efforts to assess and support patients mental as well as physical health. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. All nursing staff had completed smoking cessation training, and the practice was able to offer inhouse support and guidance.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment in an attempt to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The practice exceeded national targets for the uptake of childhood immunisations with 100% uptake across the board according to the latest Quality and Outcomes Framework (QOF) figures supplied by the practice.

The practice performed better than the England average in relation to several cancer screening programmes, including cervical cancer. The latest data showed 90% of women aged between 25-49 at the practice had been screened (against an average of 80%), whist 85% of women aged 50-64 years old had been screened (against a UK average of 66%).
