Our view of the service
Date of Assessment: 21 January 2025 to 26 February 2025. Pathfields Medical Group is a GP practice which delivers services to approximately 30,000 patients under a contract held with NHS England. The service is delivered from a main site in Plympton, and from 5 branch sites located across Plymouth in Laira, Efford, Crownhill, Weston Approach and St Judes. Information published by the Office for Health Improvement and Disparities shows that deprivation within the practice population group is in the 4th decile (4 of 10). The lower the decile, the more deprived the practice population is relative to others. This assessment considered the demographics of the people using the service, the context the service was working within and how this impacted service delivery. We assessed all quality statements across safe, effective, caring, responsive and well led key questions. At this assessment, we found the service understood their patient population and were passionate about implementing systems and processes to improve health outcomes. The service reviewed and responded to feedback and continues to improve their systems and processes. Although our remote clinical searches found gaps in medicines monitoring and we identified some issues during our onsite visit, the service addressed these immediately and implemented new processes where necessary.
People's experience of this service
People were positive about the quality of the care and treatment they received. Recent survey results, including the 2024 National GP Patient Survey and the NHS Friends and Family Test, showed people were satisfied with services. There was an active patient participation group (PPG), a group of people who were patients at the practice and represented the views of people using the service. The PPG felt listened to and they worked with the service to make positive changes, such as reviewing the repeat prescription request process at the service. We asked the service to share a link with people so they could share their feedback with CQC and 92% of the feedback we received was positive. People told us the service was well organised, efficient and staff listened, were friendly and caring.
