Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. There was information in waiting areas to promote healthy living and support services people could access. This included a mobile phone app with a list of resources to help young people and signposting to services to promote positive mental health.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service met national targets for the uptake of childhood immunisations. The service performed better than the England average in relation to screening for breast and bowel cancer. However, data from June 2024 showed uptake for cervical screening in both age groups of 25 to 49 years and 50 to 64 years, was lower than the expected target of 80%, at 73% and 74%, respectively. The service was able to track cervical screening uptake and promoted screening with people who used the service.
The service was refining their systems for monitoring care and treatment to achieve a structured approach to monitoring care and treatment. The service had implemented an Artificial Intelligence-supported tool to manage medication and long-term condition monitoring in September 2025. The aim of the tool was to integrate a safe, automated technology-based system to support people and clinicians. For example, staff used the tool to alert people and send them a link to book an appointment when monitoring tests were overdue for long-term conditions. The aim of the tool was to reduce prescribing errors by clearly flagging overdue monitoring requirements to prescribers.
The system aligned with national guidance and supported consistent clinical decision making. The aim of the system was to enable staff to see at a glance any outstanding actions, such as blood tests, medication reviews. Reception staff were able to book in blood tests which covered all outstanding tests needed, so people would only have to attend once. It also enabled clinicians to ensure all relevant checks had been carried out prior to people being seen or when repeat prescriptions were needed.
