Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to patients’ health, including those in the last 12 months of their lives, patients at risk of developing a long-term condition and those with caring responsibilities.

Staff supported national priorities and initiatives to improve population health, including programmes to assist with tackling obesity and managing diabetes.

Staff at The Avenue Medical Centre made effective use of a Focused Care practitioner to ensure people were signposted to appropriate services and received additional support tailored to their individual needs. They had provided support to patients with housing issues and helped people in need access local food banks.

Monitoring and improving outcomes

The Avenue Medical Centre routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

The practice had been unable to meet national targets for childhood immunisations and cervical screening, largely stemming from local attitudes and health beliefs regarding the screening process, this was despite a concerted education effort from practice staff, Including making more appointments available with extended opening hours and promoting immunisations both in the practice and alongside community services they work with.
