Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. They also supported people to live healthier lives and, where possible, reduce their future need for care and support. For example, the service arranged for a range of healthcare professionals to visit the practice to explain the services they offer and how patients could be signposted to them. This supported staff to better understand available options and improve access to appropriate services for patients.Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. For example, the service delivered a 12-week programme to support the management of diabetes and increased weight.

Monitoring and improving outcomes

Score: 3

The service routinely monitored people’s care and treatment to drive continuous improvement, ensuring outcomes were positive, consistent and met people’s expectations. The service met national targets for the uptake of childhood immunisations but did not meet national targets for cervical screening, achieving 76% against a target of 80%. However, performance was close to the required threshold, and the provider was implementing actions to improve uptake. For example, to reduce barriers to booking appointments, the service upskilled its team so more staff could offer the service, increasing flexibility in appointment availability. The service was also taking part in a trial of new examination positions aimed at reducing discomfort during the procedure.Clinical and non-clinical audits were carried out to improve patient outcomes. For example, the service identified an increase in patient demand and the associated pressure on appointment availability. They carried out an audit of the appropriateness of booked appointments and, through identifying a range of factors and opportunities for improvement, were able to release 29 hours per week of inappropriate clinical appointments.
