Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health. For example, they offered support to people aged under 40 years of age and diagnosed with type 2 diabetes to manage a healthier lifestyle.

The service’s waiting room and website provided information on a variety of subjects to support people to help themselves and signpost where to seek further guidance.

People had access to other services onsite such as physiotherapy, midwife services, a health coach and district nurses. This also supported communication between services to ensure people got timely support.

Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. NHS Health Checks for people aged over 40 years old were available through a central service provided by the primary care network (PCN). A PCN is a group of GP services that work together with other local health and social care organisations to provide integrated services for their communities.

The service was accredited as Veteran Friendly and raised awareness of available support services for people with a military background.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met national targets for the uptake of childhood immunisations.

Verified data for cervical screening available at the time of this assessment was from June 2024. This showed the service had exceeded the national update target of 80% for people aged 50-64 years old with an achievement of 82.5%. However, the service had not met the national uptake target of 80% for 25-49 year olds. Unverified data collected by the service in 2025 indicated they had achieved the minimum uptake target of 80% for both age groups.

The service performed above the England average in relation to several other cancer screening programmes, including screening cancer. For example, breast screening coverage for people aged 53 to 70 years old was at 80% for the year 2024-25, which was higher than the national coverage of 70%.
