Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. For example, the practice identified patients who were smokers and offered them support. They had identified 1469 smokers in the previous 24 months and offered 1466 of these support and treatment to stop smoking. They had identified 539 smokers with a long term condition in the previous 12 months and 530 of these patients had been offered support or treatment. The practice offered health checks for people with a learning disability and autistic people and had achieved a high number of these, to help ensure people remained healthy. For example, the practice had 126 patients identified as having a learning disability or being autistic and in the last 13 months, 104 annual reviews had been undertaken. They undertook NHS health checks for eligible people and had exceeded their target. For example, since November 2025 140 patients had undergone a health check against a target of 106.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met national targets for the uptake of childhood immunisations except in 1 area where they were just below the target. Staff spoken with were aware of this and consistently took action to recall patients and to speak with them regarding immunisation. Unverified data was reviewed onsite which showed the practice had achieved the 86% of their target for this immunisation so far this year and were on track to meet their target. The service performed in line with the England average in relation to several cancer screening programmes, including screening for breast and bowel cancer. Cervical screening uptake for women aged 25 to 49 years was below the national target of 80% at 69.6% according to data available to CQC. The uptake for women aged 50 to 64 years was slightly below the England target at 77.9%. Effective recall systems were in place, and flexible appointment times were offered. Additionally, the practice had outlined a quality improvement programme in relation to cervical screening, whereby they planned to implement a dedicated evening and weekend clinic and increase promotional material, with a plan to audit the outcome of these measures to review improvement.
