We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Supporting people to live healthier lives

The service always supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.

The practice worked with Health Improvement Practitioners who were non-medical, and provided services to improve patient health, by helping patients with support to stop smoking and to undertake more exercise. They also worked with patients suffering from social isolation and assisted people to attend medical appointments.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The practice had a contract monitoring lead who made calls to patients in high priority groups such as those with severe mental health issues and long-term conditions such as diabetes, to ensure they attended their health checkups.

The practice had achieved all national targets for childhood immunisations. For example, the percentage of children aged 2 who have received immunisation for measles, mumps and rubella (one dose of MMR) was 95.9% compared to a national target of 95%.

The practice had not met national targets for cervical screening, 70.9% of women aged 25-49 had received an adequate screening test within the last 3.5 years, compared to the expected national target of 80%, however the practice understood the importance of immunisations and screening and worked hard to improve uptake and educate the patients both in the practice and in the community as part of an outreach programme, as they were aware of some negative attitudes and health beliefs regarding the screening process.
