The percentage of children aged 5 who have received immunisation for measles, mumps and rubella, was 88%, slightly below the target of 90%. The service performed better than the England average in relation to several cancer screening programmes, including screening for breast and bowel cancer. The provider shared an example of a patient was experiencing confusion following a change to their steroid dosing regimen. In order to improve outcomes for the patient, the pharmacy team developed a bespoke medication chart tailored to the patient's regimen, clearly outlining doses and timings with daily tick boxes. Regular follow-up calls were made throughout the week to provide reassurance and check adherence. This meant that the patient was able to follow the revised regimen correctly and safely, improving confidence and reducing the risk of medication errors.
