Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice, and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff were proactive in identifying risks to people’s health and supporting initiatives to improve population health. The provider worked effectively with the Primary Care Network (PCN) to deliver a range of preventative and targeted services, including social prescribing, weight management programmes, ageing well clinics and a women’s health hub.
People at increased risk of poorer outcomes were identified and supported through tailored interventions. For example, ageing well clinics enabled earlier identification of unmet health needs among older people who were not routinely engaging with primary care services, supporting earlier intervention, and helping to reduce the risk of deterioration.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent, and met the expectations of people themselves.
The service used information effectively to monitor outcomes, identify risks, and drive service improvement. Performance in screening and immunisation programmes was strong, with targeted approaches used to improve uptake and access. Systems were in place to support patients who did not attend appointments, particularly vulnerable people, and the provider demonstrated a culture of continuous learning through clinical audit, service review, and shared learning. As a result, people benefited from effective, personalised care and improved access to preventative health interventions.
