Effective
Improvements were needed in relation to the documenting of some medicine reviews. Patients were involved in assessments of their needs. Staff reviewed assessments taking account of patient’s communication, personal and health needs. Staff worked with all agencies involved in patient’s care for the best outcomes and smooth transitions when moving services. People received care, treatment and support that was evidence based and in line with good practice standards.

Delivering evidence-based care and treatment
At our previous inspection, we had reviewed records of patients flagged as having a potential missed diagnosis of diabetes and those diagnosed with stage 3 chronic kidney disease. We had found some patients had not been appropriately coded within the clinical system.
At this inspection, we reviewed a sample of clinical records for patients with confirmed or potential long-term conditions. Records we saw showed that care was delivered in accordance with current clinical guidelines.
