Learning culture

At our last assessment, we found the provider had not always planned how to prevent repeated incidents happening. At this assessment, we found improvements had been made. The provider had developed new ways to analyse and record preventative action. For example, identifying and sharing lessons learnt.

The provider supported staff to learn when things went wrong. Staff reported accidents and incidents. Managers investigated these. They developed action plans to help make improvements. Managers monitored the outcomes to make sure people using the service were safe and happy with the improvements.

Safe systems, pathways and transitions

The provider supported people to have safe transitions between services. They carried out assessments to make sure they could meet people’s needs. Staff shared information with families and other professionals to ensure everyone involved in people’s care knew about their needs.

Safeguarding

The provider had systems to help safeguard people from abuse. Staff undertook training to understand these. Managers discussed different scenarios and tested staff knowledge during meetings. Staff explained they knew how to recognise and report abuse. The provider shared information with people using the service and their relatives to make sure they understood what safeguarding adults and children meant. The provider had worked with external professionals to investigate and respond to safety concerns.

People using the service and their relatives told us they felt safe and were well-treated.

Involving people to manage risks

At our last assessment, we found the provider had not always assessed or planned for risks to people’s safety. At this assessment, we found improvements had been made. Managers assessed risks and consulted with people using the service. They discussed the best way to mitigate risks and to keep people safe. There were detailed risk management plans to guide staff. These were regularly reviewed and updated.

Staff undertook training to help them understand about safe care. For example, how to support people to move and how to use specialist equipment. Managers assessed staff skills in these areas.

People and their relatives told us risks were appropriately managed.

Safe environments

The provider assessed people’s home environments to identify any risks within these. They liaised with people, their relatives and other professionals when risks were identified. They made sure equipment was safe to use.

Safe and effective staffing

There were enough staff to meet people’s needs and keep them safe. The provider had recruitment procedures to ensure staff were suitable and had the right skills. New staff completed an induction and a range of training. Managers continued to assess their skills and knowledge following recruitment.

People told us they were cared for by the same regular care workers. They told us care workers arrived on time and stayed for the right amount of time. A relative explained, “We always have the same care worker on a regular basis, and this is a good thing. The continuity is important.”

Infection prevention and control

The provider had systems to help prevent and control infection. Staff undertook training to understand these. Staff were provided with personal protective equipment (PPE) such as gloved. People told us they used this when needed. Staff had guidance about good infection prevention and control, including guides about how to clean equipment. Managers carried out regular spot checks and observed staff. These included checks on whether procedures were followed.

Medicines optimisation

People received their medicines safely and as prescribed. Managers discussed people’s medicines needs with them and their representatives. These were incorporated into care plans and risks relating to medicines were assessed. Staff were trained to understand how to safely manage medicines. Managers assessed their competencies and knowledge in this area. Staff kept records to show when they had supported people with medicines. Managers checked and audited these records. People told us they were happy with the support they received with medicines.
