Our view of the service

Date of Assessment: 7 May to 19 May 2026.

Holbrook Surgery is a GP service delivering General Medical Services to approximately 8000 people, under a contract held with NHS England. They have a branch site at Shotley. The National General Practice Profile showed 96.2% of people registered are White, 1% Asian, 1.6% Mixed, 0.9% Black and 0.3% Other. Information published by Office for Health Improvement and Disparities showed that deprivation within the service population group was in the 8th decile (8 of 10). The lower the decile, the more deprived the service population is relative to others. This assessment considered the demographics of the people using the service, the context the service was working within and how this impacted service delivery. Where relevant, further commentary is provided in the quality statements section of this report.

As a result of this assessment, we rated the service as requires improvement in safe and well led, and good in effective, caring and responsive. This gave the service a new overall rating of requires improvement.

At this assessment we found the service did not have safe and secure systems to manage medicines, their associated risks and related stationery. The provider’s governance systems did not always identify, effectively monitor and address areas for improvement we found during this assessment. However, people were involved in assessments of their needs and received effective care and treatment. Staff treated people with dignity and respect and involved them in decisions about their care.

We found 2 breaches of regulation, 1 for safe care and treatment and 1 for good governance. We have asked the provider for an action plan in response to the concerns found at this assessment.

People's experience of this service

The provider had clear information on our Give Feedback on Care (GFC) process as an alert on their website and at the service. We reviewed people’s feedback from GFC and Healthwatch Suffolk. Feedback was positive regarding the attitude of staff and the clinical care and treatment provided.

Recent survey results, including from the NHS Friends and Family Test, showed 74% of people rated their overall experience of the service as good or very good. The National GP Patient Survey showed people were satisfied with the access and overall experience provided by the service.

There was a Patient Participation Group (PPG) who represented the views of people who used the service. We spoke to representatives from the PPG who had regular meetings with the Practice Manager. They told us the Practice Manager chaired the meetings, was keen to engage and useful information was shared. This information was shared with the local community, and members were able to raise issues on behalf of people who used the service and felt listened to.
