Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.
Staff understood the needs of the local population and used this knowledge to offer advice on prevention, early identification of risk, and the management of long‑term conditions. The service provided digital resources including NHS ‘Live Well’ and a health information video library available through the service’s website. The service employed social prescribers in collaboration with the local primary care network (PCN) who connected and supported people with activities, groups and services that improved their health and wellbeing. The service promoted local initiatives used to encourage healthier living such as the ‘Bath Diabetes Wellbeing Event’ and local carers and dementia support sessions.

Monitoring and improving outcomes

Score: 3

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service met national targets for the uptake of childhood immunisations. The service performed in line with local and national averages in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer.
The service regularly monitored Quality and Outcomes Framework (QOF) indicators and demonstrated they had carried out audits to improve clinical quality and uptake to recall people in relation to their health needs. For example, the practice monitored rates of appointments that were not attended and used this information to improve service delivery. Leaders reviewed appointment use, introduced measures to reduce missed appointments and provided information to encourage attendance, including via service newsletters and social media. Nurses and healthcare assistants contacted people who were late for appointments and the service sent letters to people who did not attend appointments on multiple occasions, to ensure health needs were being met, whilst increasing appointment capacity where care was no longer needed. Staff reported improvements in attendance to appointments and people’s engagement with the service following these changes. The service planned a further audit to assess the effectiveness of these actions and compare outcomes over time.
